How care planning works

A risk assessment is undertaken by a trained and qualified person before a team member can commence their work. This will identify any potential risks associated with the provision of care, including any risks associated with medication and other health-related activities.

The risk assessment includes an assessment of the risks involved in helping the client maintain their independence and stay safe in their own home. The views of the client — and where applicable, their family or representatives — are taken into account at every stage.

Reviewed every three months

The risk assessment and care plan are reviewed every 3 months, or sooner if anything changes. You'll be involved in every review.

A care plan you can actually see

A plan to manage risks is drawn up in consultation with the client. A copy is available to view alongside the care plan — for the client, family members, and the team member providing care. Real-time information is also available through our Birdie call monitoring system, so families always know what's happening.

The review cycle

We don't set care plans and forget them. They're living documents, reviewed continuously:

Full care plan and risk assessment review with you and your family.

Every 6 months

Client satisfaction survey — your chance to tell us what's working and what isn't.

Ongoing

Spot checks, team supervisions, and feedback forms throughout the year.

Any time

If anything changes — you, your needs, your family situation — we adapt straight away.

What it costs

Charges are calculated based on the care provided and the number of hours worked. They are subject to review from time to time, and clients are always notified in writing of any variations before they take effect.

We don't publish a single rate card because no two care plans are the same. Our pricing is built around what you actually need — not pre-packaged bundles. We'll talk you through it openly during your initial conversation and home visit.

What's included in every package

Uniform and PPE for every team member, mileage and travel time, full insurance, 24/7 on-call support, and the Birdie monitoring system at no extra cost.

Care works best when the same familiar faces show up at the door. We work hard on this. Regular communication and continuity of carers are central to how we operate — it's one of the things that sets us apart from larger, more impersonal agencies.
